Evaluation Form - Health & Renewal Proposals

This is the form that the SCE office uses when evaluating a proposal. You may find it helpful as you prepare answers for the application form.

#1b    Are two congregations partnering in this project?
           If yes, the two congregations are:    
           1.
           2.         

          Does the congregation(s) meet the size criteria?
          How many worshippers on Sunday am?

          Does the congregation participate in classis meetings?
          Does the congregation participate in Minister’s Pension Fund?
          What percentage of Ministry Shares is contributed?

#1c:   Does the church have a mission statement?    
          Does the church have a vision statement?

#1d:   If no to above questions, is this question sufficiently answered?
     
#1e:   Can this be realistically accomplished by the congregation?

#1f:    Is the focus of the project clearly stated?

#1g:   Is there a wide representation of church members in the list?

#2a:   Does the description give a clear picture of the project?
 
#2b:   Does this project appear to foster health in the congregation?

#2c:    Are the goals clearly stated?
           Are the goals realistic?
           Do the goals line-up with the project description given in #2a?
           Are there too many/not enough goals?
           Do the activities truly demonstrate commitment to the goals?
 
#2d:   Are indicators of the project’s success clearly stated?
           Are they quality indicators?
      
#2e:   Has the congregation(s) already done something that demonstrates a commitment to the 
           goals stated in #2d?
  
#2f:    Which of the practices/characteristics is the project linked to? 

#2g:   Is the timetable complete?
           Is it realistic?
           Are events/activities taking place after Feb 1/Aug 1?
           What is the length of the project?

#2h:   Is the church requesting an SCE coach?
           If yes, what will the coach focus on?
           Is this a key piece of the proposed project?

#3      Budget narrative: 
           Are expenses explained adequately?  
           Are expenses reasonable (a fair price for the item)?  
           Are expenses justified (is this expense necessary for the project)?  
           How much of the total is for equipment? 
           Does the budget appear reasonable?      

           Are there any in-kind funds noted?
           Is there a sustainability plan in place?
           If yes, does it appear feasible?

#4/5:  Are the coordinator and treasurer immediate family members?

#6      Has council approval been obtained?

#7      Has the overview document been read by 2 ministry leaders?
