Evaluation Form - Coaching for Pastors Proposals

This is the form that the SCE office uses when evaluating a proposal. You may find it helpful as you prepare answers for the application form.

#1:      Is the information complete?

#2a:    Does the congregation(s) meet the size criteria?
            How many worshippers on Sunday am?

            Does the congregation participate in classis meetings?
            Does the congregation participate in Minister’s Pension Fund?
            What percentage of Ministry Shares is contributed?

            Does the church have a mission statement?   
            Does the church have a vision statement?

#2b:    If no to above questions, is this question sufficiently answered?
     
#2c:    Can this be realistically accomplished by a congregation?

#2d:    Is this a realistic definition?

#3a:    Has this question been clearly answered?
 
#3b:    Are the expected outcomes realistic?

#3c:     Does it appear the coaching will foster health in the congregation?
 
#5:      Has council approval been obtained?

#6:      Has the overview document been read by the pastor?
