Evaluation Form - Coaching for Churches Proposals

This is the form that the SCE office uses when evaluating a proposal. You may find it helpful as you prepare answers for the application form.

 #1a    Are two people listed?
            If the congregation currently has a pastor, is one of the people the pastor (required)?

#1b     Does the congregation(s) meet the size criteria?
            How many worshippers on Sunday am?

            Does the congregation participate in classis meetings?
            Does the congregation participate in Minister’s Pension Fund?
            What percentage of Ministry Shares is contributed?

#1c:    Does the church have a mission statement?    
            Does the church have a vision statement?

#1d:    If no to above questions, is this question sufficiently answered?
     
#1e:    Can this be realistically accomplished by the congregation?

#2a:    Which type of coaching is requested?           Leadership             Process
 
#2b:    Is a sufficient answer given?

#2c:    Is the answer realistic?         
            Should the expectations be higher/lower?

#4       Is the grant coordinator one of the 2 people listed in #1a?
 
#5       Has council approval been obtained?

#6       Has the overview document been read by the 2 ministry leaders?
