Faith Formation Survey

The survey on page 12 of the Walk On Resource Book was intended to help a congregation identify and focus on its strengths and weaknesses in faith formation. The survey is primarily intended as a discussion starter—not a scientific assessment tool. The scores should only be used to help your members discuss the aspects of faith formation that are working well and the areas that could be improved.

Here are some variations of the Walk On survey that may work well in your church:

  • Remove the 1 to 5 scales on the questionnaire. Use the statements to help your congregation discuss areas of strengths and weakness in faith formation.
  • In order to make the survey more clear, the mid point of the scale could be changed from “no opinion” to “neither agree nor disagree.”
  • A separate option of “no opinion” could be added or people can simply be asked to omit answering any questions where they do not know the answer or have no opinion.
  • Rather than handing out the survey, use the statements in the survey to lead a discussion on the topics that you wish to explore with your church group.
